We would like to have you fill out the three enclosed forms for us:

1. Patient Information/Insurance form

2. COMCARE New Patient Health History Form

3.   Authorization for Release of Information Form

4.   New Patient Survey

Our New Patient Counselor is located at 520 S. Santa Fe, Suite 300.  You may bring your completed paperwork to any location or return it in the mail with the enclosed self addressed envelope.  If you have questions or difficulty completing the forms please contact the New Patient Counselor at 785-823-4346 so that she may assist you.
